A caregiver’s journey: It can be overwhelming, confusing and filled with crisis. Advocate For Elders services are available to older family members to alleviate the uncertainty faced by caregivers.

- Assessment and Care Plan
- On-going monitoring of services
- Power of Attorney/Conservatorship

Assessment and Care Plan
- Conduct assessment, which involves the caregivers/family and care recipients. The evaluation of the care recipient’s physical, mental, social and home safety status provides a valuable framework for planning.
- Compile an individualized care plan that supports the strengths and addresses needs of care recipient. Plan includes recommendations of services and resources related to housing adaptations, housing alternatives, home health services, chore services, social connections, legal and financial planning.
- Guide decisions and arrange for services identified in the care plan.
Routine Monitoring of Care Recipients
- Monitor care by visiting clients residing at home, assisted living facilities or nursing homes.
- Routinely review implemented services including home care, household maintenance, medical, adult day care, legal and financial.
Power of Attorney/Conservatorship
Coordinate care-related services for older adults; manage, preserve and direct financial assets towards support of care.
Advocacy
Encourage families to discuss their social, financial and ethical concerns and wishes before a crisis situation. Conduct one-hour seminars – The Other Important Talk: What Boomers And Their Parents Need To Discuss Before It Is Too Late!
